PatientWise is a patient information portal within the PrimeWise information sharing platform, developed and supported by RAPHS. Use of PatientWise is restricted to authorized individuals and only for the purpose of providing a clinical service to patients in their direct care, and as per conditions specified in the User Access Agreement signed by that individual
