  • recall useful ACP conversation starters
  • explain how to respond appropriately to patients during ACP conversations
  • explain what to do at the end of a conversation
  • make a plan to raise ACP with your patients

  • define key terms, including decision-making competence, valid consent, advance directives, enduring power of Attorney
  • explain how an advance care plan can influence a clinician's decisions and change a patient's outcomes
