Whiri Model of Care

Whakamārama| Description: This is a comprehensive learning journey about the Whiri Model of Care. This model of care is reshaping how we support whānau and other high-risk communities across New Zealand. Whether you're a Navigator working directly with whānau, a clinician providing frontline care, or are part of the leadership and governance teams, this programme will give you all the skills you need to implement Whiri into your service. Beginning with Whiri 101, you will learn the foundations of the model. Building on that foundation, you'll then explore how to use guides like the Whānau Hauora Assessment Tool (WHA), build strong referral pathways and apply continuous quality improvement practices.
